MARPE
The MARPE procedure works by expanding the upper jaw (maxillary bone). When the expansion is complete, it increases the space in both the nasal cavity and oral cavity. This helps reduce nasal resistance and allows the tongue to sit higher and more forward in the mouth, which can improve airway size.
The treatment involves placing a custom maxillary expander with small mini-screws along the mid-palatal suture. The procedure usually takes about 20–30 minutes and is performed under local anesthesia. Any discomfort afterward is usually mild and can be managed with over-the-counter pain medication for a few days. After the device is installed, the patient turns a screw daily for about 4–6 weeks to gradually expand the palate.
The expander separates the mid-palatal suture, creating a small gap between the front teeth. Orthodontic treatment can then close this gap and help establish a stable bite. The expander stays in place until new bone forms in the suture, which generally takes about 4–6 months.
Compared with traditional expanders, MARPE has fewer side effects and can be used for a wider range of patients. It uses mini-screws to apply force directly to the bone rather than the teeth. Traditional expanders attach to the teeth, which can cause unwanted tooth movement. MARPE focuses on expanding the bone itself, which helps maximize airflow while minimizing side effects.
While MARPE can be an excellent option for treating sleep apnea, other treatments may also be considered. For mild sleep apnea, doctors may recommend lifestyle changes such as losing weight or quitting smoking. Patients with nasal allergies should address those issues first. If lifestyle changes do not improve symptoms, or if sleep apnea is moderate to severe, other treatment options are available.
